The Gift Of Animal Life Application Guidelines Organizations or individuals wishing to be considered for funding by The Gift Of Animal Life Sanctuary, Inc. (GOALS) should first carefully review GOALS program guidelines and objectives. The objectives of The Gift of Animal Life Sanctuary (GOALS) are: ¥ provide safe homes for animals ¥ strengthen organizations who serve animal needs ¥ to provide assistance when no other source of assistance is available ¥ reduce unwanted animal pregnancies ¥ advance the understanding of animal needs ¥ to create an environment of care and support for animals ¥ to promote a higher level of understanding of the need to care for animals ¥ to develop a sanctuary for animals. GOALS has no restrictions on the type of support it will consider so long as an organization's or individual's work is consistent with GOALS' objectives and interests and it is permissible under applicable U.S. charities law. GOALS primarily makes grants to public charities classified as tax-exempt under section 501(c)(3) by the U.S. Internal Revenue Service (IRS), but will consider other organizations and individuals, in the U.S. and elsewhere, if their purposes are consistent with those established by the U.S. IRS for not-for-profit organizations. We do not require letters of inquiry from those organizations or individuals wishing to apply for grant funds. There is no set limit on the number of applications GOALS accepts. There is no preset requirement that an applicant provide a cash or in-kind donation of labor match. GOALS awards support based upon the applicants ability to demonstrate that the activity funded by the grant supports the stated objectives of GOALS and that the applicant can demonstrate that they can successfully complete the project or activity for which they are requesting support, and can measure the impact of the activity and report back to GOALS. Applicants should be aware that incomplete application forms or applications with projects or purposes other than those stated in GOALS grant objectives will be discarded and not returned to the applicant. Applications are not kept beyond the three-month window of application submission and deadline. If you are not awarded a grant, you must resubmit your application within a new three-month window. All grants awarded will be posted on our Internet web site and applicants who receive support will be notified by mail. Applicants whose applications were not awarded grants will not be notified. You must check the Internet site to determine if your application was funded. Applications are accepted at any time and grants are awarded two times each year, on the first day of each quarter. The application and the supporting documents should be as detailed as possible and long enough to adequately make the case for grant funding. Applications are limited to ten pages in length, including all attachments. Applications with excess attachments will not be considered for funding. The Gift of Animal Life Sanctuary Inc. funds both organizations and individuals. The application form may request information that an individual may not have, (such as "title" for the authorizing officer). We ask that individuals fill out the information to the best of their abilities and indicate where applicable that information requested of organizations is not applicable (N/A). 1. Organization or Individual Information a. Fill in the name of the organization, or if an individual applicant, your name. b. List the address at which you receive mail. c. Indicate your city, state and postal or zip code and your country. d. List the name of the person whom you wish GOALS to communicate with. e. List the telephone number of the person we should call if we have questions. f. List the e-mail address of the contact person and a web site if there is one. g. Indicate the type of organization applying for funds - individuals indicate their status as "individuals". h. List only U.S. Federal Identification Numbers - do not list Personal Social Security numbers. i. Indicate the type of organization or activity that this grant supports. 2. Provide the mission of the applicant organization or, if an individual is applying, a one-sentence description of your work associated with the grant application. 3. Describe the project this grant supports in one sentence. 4. Describe how your project addresses the stated objectives of GOALS ( advance the understanding of animal needs) 5. List the primary activities that this grant supports (i.e.: education, prevention, capital expense, neutering, etc.) 6. Write your narrative. We ask that it be typed or computer generated but we will also accept handwritten narratives if they are legible. Go down the summarized list and fill in the requested information. 7. Attach a copy of your organization's financial statement and indicate if it is an audited financial statement. Individuals should indicate their adjusted gross income from the last year but not provide copies of tax forms. Organizations or individuals residing in countries other than the U.S., provide suitable information and write a description of how the information is calculated in their country. 8. Attach a list of current the applicant organization's board of directors and include their titles (executive director, etc.) and their experience associated with the organization ( a local veterinarian who donates service to the shelter, etc.). If you are an individual applying you do not need to attach anything. If you are an organization from a country other than the U.S., attach a copy of your executive committee or other officers. 9. Attach a copy of the applicant organizations's IRS tax exempt certificate (the letter indicating that the organization has been recognized by the U.S. IRS as a not-for-profit entity). If you are an individual you do not need to attach anything. If you are an organization from a country other than the U.S., do not attach anything unless you have documentation from your country's government indicating your status as a public charity or other not-for-profit. 10. Count your pages. If you have less than ten pages you may submit letters of support or copies of photographs. We ask that you describe what it is that you are attaching. Do not attach original photographs or other documents that you would like returned to you. We will not return documents or photographs. 11. Read the certification. Do not sign the application unless you understand and agree with the text of the certification. Provide your title within the applicant organization if you have one. If you are an individual applying for assistance write "N/A" in the area requesting a title. Print your name and date the application. Mail your application with a U.S. postmark. Note the application deadlines. If you are submitting an application from a country other than the U.S. we require your application to be received within three business days of the application deadline date.
